Please note: Only your designated accounts payable contact, also known as the “Billing Administrator”, has access to download W-9 forms on the Customer Payment Portal. If another contact in your practice should have access to the Customer Payment Portal but does not, please contact our Support Team at gSupport@modmed.com, leave a voicemail at 561-880-2998 x3, or create a case on ModMed® Communities for gGastro®.
