2024 MIPS Updates & 2025 Final Rule Highlights

We are writing to inform you of important MIPS-related updates and deadlines as the end of the year and the 2024 MIPS Submission period approaches. We recommend that you review your MIPS Dashboard to note your current score and consider which measures you will submit. Also, please visit our 2024 gGastro® MIPS Guide, available on ModMed® Communities for gGastro, for detailed information.

Important Deadlines & Details

Please make note of the following important deadlines:

    • Tuesday, December 31, 2024: End of the 2024 MIPS Performance Period. Please be mindful of the following caveats:
      • The Quality Category for MIPS must be reported for a full calendar year (CY) per Taxpayer Identification Number (TIN).
      • MIPS Promoting Interoperability data can be reported for at least 180 consecutive days within a CY (meaning you can choose to report for a longer period if desired, but the reporting period must be at least 180 continuous days).
      • Improvement activities have a minimum of a continuous 90-day performance period during CY 2024 unless otherwise stated in the activity description.
  • Monday, January 2, 2025: Start of the submission window for the 2024 performance year (PY). 
  • Monday, March 31, 2025: The Centers for Medicare and Medicaid Services (CMS) closes its submission window.

The 2025 Final Rule

On Friday, November 1, 2024, CMS released the 2025 Quality Payment Program Final Rule for MIPS, which includes the rules and regulations for MIPS and MIPS Value Pathways (MVP) for 2025. Additional details are forthcoming. In the meantime, please be aware of the following highlights:

  • A new Gastroenterology MVP has been introduced for the 2025 Reporting Year.
  • Cost benchmarks/methodology have been updated to assess clinician cost of care more appropriately, using mean and standard deviation rather than average. 
  • Changes have been made to Improvement Activities (IAs):
    • Four IAs have been removed from 2025 reporting and four IAs have been removed from 2026 reporting, including:
      • 24/7 Access to MIPS Eligible Clinician
      • Implementation of the PPE Plan
    • IA weights have been eliminated
    • One IA is required for Small Practices, Rural, HPSA, and Non-Patient Facing practices.
    • Two IAs are required for Large Practices of 16 or more clinicians.
  • The 2025 Performance Threshold will remain at 75 points to avoid a negative payment adjustment.
